describe the employment. Include the name of the person employed, name and
address of the employer, weekly income and whether the employment was specifically
authorized by USCIS.

name of the person employed: Employee Name, Teacher
name of the employer: Employer name.
address of the employer: employer address
Part Time H1B
weekly income: weekly salary:$260 is ok?(Requirement weekly salary:$500
)
whether the employment was specifically authorized by USCIS? "Yes" is ok?

I need provide more information on this Question ?
