what is the approval message for 485?

I saw people get:

- approval notice sent
- card production ordered
- card mailed

are we supposed to get all of them or not always? I quickly checked a few old cases approved in 2006. their status are approval notice sent. but I think they should have received their cards long time back.

so is this related to the people who process cases or there is no formal way as such? thanks.
