You'll have to send following documents to the place that issued you the card with error.
1. I-90 Form. Make sure check the item that indicates that it's an administrative error, so you don't need to pay.
2. The original card with the error.
3. A cover letter that explains the error.
4. All the supporting documents (passport, birth certificate, driver license, 485 receipt...) in copies.
5. The page with the barcode that comes with your card.
You will receive a notice about 10 days after they receive the package. Many people say it would take about 3-6 months to get the new card, but it only took 3 weeks in my case. Very impressive. You can make an interview through INFOPASS to get a I-551 stamp on your passport before you send out the package, but many local USCIS officers don't do this for you.
