question about I-485, thanks!

I am preparing to apply EB-1A, and experts here told me to submit I-140, I-485 and labor certificate application forms together. I just go through the form I-485, and part 2 is asking the application type. I suppose that I should check the first box for "an immigrant petition giving me an immediately available immigrant visa number has been approved. (Attach a copy of the approval notice, or a relative, special immigrant juvenile or special immigrant military visa petition filed with this application that will give you an immediately available visa number, if approved.)".

My question is: I submit I-140 and 485 together, and no way to attach a copy of the approval notice. So, what can I do in this case?
My second question is : what form I should file for labor certificate application?
My third question is: Are the addresses for I-140, I-485 and labor certificate the same one? and could I put them together in the same box and send it out to the same address?
