if 2 is b, yes, two (or more) approval notices with this employer.

No I-129 at hand, so not sure what exactly the questions are, page 3 I assume one is about immigrantion petition, you should answer yes (because of YOUR EB2 I-140)

page 10, I don't have the form at hand, so no comments on the yes/no a/b/c,.. but the Japan PhD part, it seems to be the US Master degree 20K number screener. If you don't have a US master degree, then you don't use the 20K number. In fact you don't use any new number at all because of extension.
