renew AP用提交I94 back copy嗎

On I131 instruction, it only says applicants need to provide evidence of their current status, e.g. I94 or I797 etc. Didn't say for I94, need to provide a back copy of it, which in my case nothing on the back. I also have a H1B extension approval notice (I797), which is the latest evidence of my current status, date on I94 is passed already. Can someone who has done so please share the experience? Want to send out the form tomorrow. Appreciate your help.
