online fill 156, download 157, go to websit of US embassy in Beijing, there is detail information about that.
Bring your passport, H1B approval notice (original), receipt of the fee you paid (780 yuan), I also brought a employ letter (say I'm a employee of XX, title, start date, salary, current H1B visa valid tillXX), that's it.
