•Print of the receipt of SEVIS I-901 fee ($100.00);
•Form I-539, “Application to Extend/Change Nonimmigrant Status”;
•Mailing label as required;
•The latest, original Form I-20 from University of XXX;
•Copy of the initial admission and offer letter for a doctoral assistantship;
•Copy of the latest letter from my college to show its continuous financial support;
•Copy of my graduate assistantship contract for the Fall 2007;
•Copy of my current H-4B approval notice;
•Copy of my wife’s current H-1B approval notice;
•Copy of the marriage certificate with translation; and
•Copies of all my other I-20’s, EAD, and my latest entry visa.
