you can go to any notary office to get the blankform for fingerprint, here in L.A. it's $3 each, and $15 for taking the fingerprint. You don't have to request that from FBI, it's too slow. I can send you that if you can not get that from your local notary office.

BTW, I'm going to apply for Canada immigration, do you have any idea for the following questions:
1. What time should I get the police certificate ? Can I send that along with all other forms and documents, or have to wait until the immigration officer.
2. What is the proof for working experience ? "Notarized contracts"

I want to do itmyself, there are something not very clear to me, it would be great if you could give me some information, really appreciate it.
