New Chinese passport question.

My current passport is about to expire, so I need to apply for a new one. Pjiang's blog says I need to fillout form H1. I am in Los Angeles. It seems they are using different form BG02. Plus they only ask for 3 photos instead of four. I'm really confused now. Which one I should use? Thx
