Xiao Baitu, Pjiang, Radiology, please help, urgent!

It is almost 3 months since I filed I-485. But I have received nothing and my checks have not been cashed until today. I want to make an enquiry to USCIS about my case. Could you please suggest what documents/information I need to send out with this enquiry. I have only I-140 #, I-94 # in my hand.
