If one's labor is filed with less than 365 days remaining, then the only way to get extension is labor approval and 140 approval.
If the labor is filed with more than or equal to 365 days left, then whether the labor is approved or 140 pending or 140 approved, one is eligible for H1 extension (the only difference is if 140 is approved, the extension would be for 3 yrs; if not, the extension would be for 1 yr).
So far, the law hasn't changed with the introduction of PERM processing.
Bottomline : If you applied labor with less than 365 days left, then you need to have labor approval + 140 approval to gain eligibility for H1 extension.
This is my knowledge after reading many forums and also after talking to my attorney.
