Because of your limited information, assumptions were made - 回複：

1) you filed I-539 to change to H-4 before your current status (say H-1) expired
2) you filed I-129, trying to be back to H-1, after your H-1 expiration while your H-4 is pending
3) you used premium process on I-129, which caused a decision on I-129 earlier than I-539

As your H-4 hasn't yet approved, your did not meet requirements of COS, when filing I-129, which are
1) 當前身份沒有違例；
2) 申請在當前身份有效時提交。
although your specific situation may be excused at USCIS' sole discretion, apparently they show no mercy on you.
