How can H-1B be renewed after the 6th-year next July, working on

My friend's H-1B will reach its 6th year maximum next July, July 2008, never applied for green card, working with an attorney for Labor Certification (LC) not yet filed. Questions:
1. Can the H-1B be renewed if LC not yet approved by next July?
2. How can she work in USA after next July if LC not yet approved by next July?
3. At what point, can the H-1B be renewed during the LC pending (filed, but not approved)?
