Thanks Bioinfo. But your sample is the same as that in "Non-prisoner" packet.

I'm from LA, California. On CA Federal court web site, they provide both "Civil Case Pro Se Package" and "Civil Action Non-prisoner Package" download. And the form in "Civil Case Pro Se Package" is similiar to the sample cover sheet on your web site.


This is really confused me.
