when you apply for the case #, there are two adresses , one for sending money order, and the other for sending the application form. if you send those two items to the wrong places, it will take a long time. for the phone, if you choose the right options, you will be able to talk to someone at DOS. I do not remember how to do it exactly, however, I did get to talk to some one before. Good luck.
