6年H1B延期DIY，有幾個問題請教各位老師
1，Part 2 Q3,If you checked Box 2b, 2c, 2d, 2e, or 2f, give the petition receipt number:
這裏是不是填我5年前第一個H1B 的 RECEIPT WAC NUMBER ？

2,Part 2 Q4,Prior Petition. If the beneficiary is in the U.S. as a nonimmigrant and is applying to change and/or extend his or her status, give the prior petition or application receipt #:
這裏是不是兩年前的第一次延期的H1B的 RECEIPT WAC NUMBER ？

3 Part 4 Q9，Have you ever previously filed a petition for this person?
因為一次H1B和一次延期公司職務都沒變，是不是填 YES，ATTACH什麽文件，兩個H1B I-797的COPY嗎 ？

4，Part 5,Q3/Q4 LCA Case Number/NAICS Code
LCA Case Number是不是就是Labor Condition Application 的那個ETA CASE NUMBER，還是Occupational code? NAICS Code是什麽東西呢？

5，Part 3 A# 必須填485 RECEIPT上的那個A#嗎？

6.Shall I put my families I-539(H4 extension) together with my documents in same envelope and ship it to the same office?
