急！485 被退，請分析原因！

我女兒的485昨天被退回來，原因是485表格沒有簽名。因為我女兒隻有四歲，所以全部是我代填表，我在填表者項簽名，申請者欄就什麽都沒填。不知為何被退回來了。請教版主和各位大俠，怎樣才算符合要求。急。多謝。
