just enclose either your I-140 approval notice or the cover page of yoru L/C. If your L/C is under EB3 and I-140 has been approved, you can extend for 3 years if you file H extension in September since visa number is not available to Chinese under EB3 in September. If I-140 is still pending, you can extend for only 1 year.
