Passport question.

Do I need to bring the orignal copy of the whole I765 (for H1b) or only the bottom portion is enough for passport change (getting a new passport according to the new policy in Chicago next week).
If the whold original I765 is needed, I need to get it from the internationnal office in my university (troublesome).
