Questions on I-485

I have two questions on filing I-485 form.

1. which I94# should I fill out, as I changed my visa from J1 to H1 recently.

2. which visa information should I use to fill out the form, J1 (previous) or H1 (current)?

I really appreciate any suggetions and comments!!!
