The Form I-693 must be properly endorsed by the civil surgeon to reflect all of the following: 1) The date you were referred to the local health department for further evaluation, 2) The name, address and telephone number of the local health department, 3) The results of the X-ray and any other diagnostic tests for TB, and 4) The basis upon which you have been medically cleared for adjustment of status.
