3. I came as F-2, changed F-1, H-1 and just changed H-4. Should I put Visitor or Student? I went to Canada for F-1

4. Date of entry into U.S. -- should it be my last date of entry, or first entry date?

5. We just bought a house. They require the date we plan to live. Should I leave it blank or put the date my hus's H-1 expiration date?

We have moved several times and I never done the form yet. Should I insert a letter? What should I say? Apologize or else.
