請問I-765表第11項，關於以前得到OPT

以前申請過OPT並得到EAD 卡.
問題是我沒有保持批準的信件,也不知道是那家USCIS Office,何時批準的.
我根據那個EAD卡上的Receipt Number",在USCIS網站上查case status seach. 結果顯示說"On September 28,2005, we mailed you a notice that we approved this I765 Applicaiton for employment Authorizaiton. .......

把這個打印出來可以嗎? 是不是可以填September 28,2005為批準date?
