背景調查和FBI - FOIPA

I-485 received in May 2006. Faxed FOIPA request to FBI but got returned. it says in letter：

Your Freedom of Information-Privacy Acts(FOIPA) request is being returned. Your letter did not contain sufficient information to conduct an accurate search of the central records system at FBI Headquarters. The following information is necessary ....

The items listed below are exactly the form I got from the FBI web. I filled in all items to fax them. Does anybody know why it was returned? Do they accept fax? Do I have to mail the request to FBI?
