I asked this q before so i can tell u the answer

Actually, based on some samples, u can extend h1b after 6 yrs. However, since the 485 is filed, it said u can only get 1 yr extension. (if u do not file 485 and ur 140 is approved, u may get 3 yrs extension). However, there are still some samples which get 3 yrs extension even 485 is filed. Therefore, u can file extend h1b after 6 yrs. The only concern is how many yrs extension u got.

Regarding whether u need to have approval of 140 to extend h1b after 6 yrs.. Actually, i do not think it necessary
