請問Pjiang，小白兔和放射學等各位高手: How to file 485 with my I-140 receipt

Thanks very much for answering my previous questions. I really appreciate it.

I have my NIW I-140 receipt, so when I file 485, should I just give them:
my name
copy of my I-140 receipt
I-485 forms

Is that all? Thanks in advance!
