Apply as H4 should be.

1) H4 can study and this is what you are doing now. Make sure you didn't get an I20 from your current school and your name is not on SEVIS.
2) The questions of "Do you intend to study in US ?" meaning if you want to study full time and on F1 status. If you plan to switch to F1 status, then say yes. If you are not, then say no. Even you can take full classload and thought you are studying full time, but officially H students are still part time because you are not on F1 status.
