XBT or experts, how to fill in I-94 when using AP to enter US

I am going to use AP to enter US. When I fill in I-94 form (arrival record), how I should fill in the blanks
' 8.country where you live',
'10. city where visa was issued' and
' 11. date issued'.

Appreciate your help and time.
