usually people should turn in the receipt while departure (if the B-2 extension is still pending). In your case, i have no idea.. sorry about that. But seems to me, it's no big deal. Either way, your dad will be just fine. He can keep this approval with him together with the air tickets and show it to VO next time, it will explain everything. If i were you, i probabely will turn the official approval to local INS and ask them to correct the records saying that your dad's already left (you probabely need to show them the proof of when he left, and etc.)...
