My case:
I came to usa four years ago with a J1 visa issued in Shanghai(in the year of 2003) from the entry port of LA and the visa was transfered into H1B in 2003 in USA. Last year I visited Canada for few days and come back to USA via the entry port of BLA without a new visa last year but a new I-94.
Questions:
What is my last entry, LA or BLA?
What is the date of my visa issued, the on in 2003 or 2005?
What is the Consulate where your visa was issued?
If I put the BLA as the place of my last entry, but Shanghai as the place where my visa issued, it seems they do not match.
I would greatly appreciate it if anybody could response these questions.
