1。請問 2001年 的 PPD POSITIVE 的報告是否過期了？ 我那次作皮試時把腫塊部分蹭破了，好幾年痕跡才消失，這次不想再做皮試了。

2。學校是把報告 FAX 過來的，請問傳真件可以用嗎？還是要必須通過信封郵寄的複印件才行？
it depends on if your civil surgeon would recognize it.
as long as he's willing to put the size & date on 693 form, it's fine. the civil surgeon might need the original copy.

問題3。我預約的 CIVIL SURGEON 讓我自帶 I693表，請問我是否從網上打印一份就可？

yes. please notice that there are several copies (for cis, doctor, yourself, etc) of 693 forms, also supplement form.
4。我可以用外州的 CIVIL SURGEON 做體檢嗎？還是必須用自己所在的州的醫生？
out of state is fine
