1) In what status did you last enter? (visitor, student, ..........)

Mine is H-1B visa. Should I put "H-1B" here?



2) Were you inspected by a US Immigrant Officer?

Should I choose "Yes"?

3) Nonimmigrant visa number?

Which number is this? Is this H-1B receipt number, starting with WAC-04-201-50000??


4) Consulate where visa was issued?
My H-1B was approved by INS in Laguna Niguel, California. Should I put Laguna Niguel?
