1. Complete copy of current passports (including blank pages) as well as expired passports if they contain any U.S. visa(s).

2. Copy of the front & back of the I-94 cards (the cards given to you at your last entry into the U.S.).

3. Current verification of employment letter from xxx (original and contain Salary).

4. Six (6) Immigration photos. Please refer to http://travel.state.gov/passport/guide/guide_2081.html for guidelines.

5. Medical Exam. (to be performed by authorized doctor). Please bring records of previous immunizations to your medical appointment. You can locate an authorized doctor by visiting:
https://egov.immigration.gov/crisgwi/go?action=offices.type&OfficeLocator.office_type=CIV) The results will be given to you in a sealed envelope. Please do not open the envelope as it will invalidate the exam.

6. Copy of your US tax return and W-2 for 2006

7. Copy of certified birth certificate or family census register. The USCIS requires that birth certificates must state the date and place of birth and full names of both parents. (In some countries this is the form of birth record referred to as "full birth certificate" or "long-form birth certificate").

8. Copy of your marriage certificate. If you or your spouse have been married previously, we will need a copy of the certified divorce decree(s)

9. Copy of all current & previous USCIS approval notices/documentation (such as: previous H/L approval/extension notices (Form I-797's), copy of all previous I-20's (if ever in F status), copy of any EAD's (if held practical training, L2/J2 work authorization), IAP66's (if held J status).

FAQ's:

1. Once you gather the majority of the documentation, please ASAP and give us a cover sheet of the documents you are still working on (such as birth certificate or affidavit), this will allow us to begin preparation. Please also answer every question in the questionnaire. Your answers in your questionnaires will need to match those names/dates on your marriage certificates and birth certificates. Please do send the entire copy of your passport(s) including blank pages.

2. We have hundreds of people who will be eligible to file, please be patient and do not request status of your file. The cases will be prepared on first come first serve basis. The next step once you your paperwork is a draft of your application which you will receive by email.

3. If you have travel plans outside the US in July, please NOTIFY us ASAP, You and your dependent family members who are filing Adjustment of Status (I-485) MUST BE IN THE US on the date the adjustment is filed.

4. As of now, visa numbers will only be current in July, each case takes a couple of weeks to prepare. We must be prepared to file each case before July 30, 2007. We cannot wait until July 30 to file all the cases, so please turn your information in ASAP.

5. You are eligible for the adjustment of status application if the I-140 is pending.

6. If you are from India and you were born after 4/1/1970, a birth certificate should be available to you. If one is still not available, then you will need to collect a statement from the local government authority that confirms they have checked their records and your birth certificate does not exist or is not available. You would also need to collect two affidavits of birth, preferably from your mother & father.

If your birth certificate does not state your name, but indicates "male child" or "female child," you will need to obtain 2 affidavits of birth to accompany your birth certificate.

7. The application will include a request for an EAD and travel document (Advance Parole) for your spouse. Once your spouse receives the EAD (employment authorization documents) she will be eligible to work in the US and if she works, she will need to travel into the US using the Advance Parole document and should not leave the US until she physically receives the advance parole.

8. If you are NOT in H or L status, you CANNOT travel outside the US after the Adjustment of Status is filed unless you have Advance Parole in your hands. This generally takes approximately 3 months, and may take longer as there will be a significant number of filings.

9. You do not need to complete a questionnaire for a U.S. citizen child or spouse.
