485和G-325A填表的問題

485裏：
1. In part 2, Application type, should I choose a)?
a) An immigrant petition giving me an immediately available immigration
visa number has been approved.

2. If my wife files I485 together with me, should he choose application type b)?
b) My spouse or parent applied for adjustment of status or was granted
lawful permanent residence in an immigrant visa category that allows
status for spouse and children.

3. In part 3
Were you inpected by a U.S. Immigration Officer? Yes or no?

4. My wife came to US as H4, change to F-1 then H-1B in US, and has never left US since she came.
In Part 3, does "Nonimmigration visa number" mean H4 visa number? Do "the consulate where the visa was issusd" and "date visa was issued" all refer to her H4 visa?
