7) Marriage Certificate (AOS)

8) Medical Exam Results (AOS)

Must be in a SEALED envelope, for each family member.


9) Nonimmigrant Visa (AOS)
10) Passport (AOS)

Please provide a copy of all pages (even blank pages) of your passport.


11) Passport of each Family Member (AOS)

Please include copies of the entire passport, including the blank pages.


12) Photos (AOS)
