請問 I-539 form

Experienced please help. On I-539 form for H1 transfer to H4:

Is the original I-94 required to submit? Or the copy will be OK?

The blank in Part 1: Address-in care of- What does this mean? How to fill?

The 1st question in Part 3: I request that my current or requested status be extended until.....Since I am filing a transfer to H4, what to fill? Should it be my spouse's H1 expiration date?

Is the mailing label needed?
