Yes you need both documents. For the birth cert. you can get from "gongzhengchu". They issue that document based on your "hu kou ben". They will also translate and put steal seal on the cert.

For the vaccination, you can either use your yellow book when you left China after the physical exam or take all the shots in the US.

Fortunately I kept my little yellow book from China and the doctor from INS authorized clinic only gave me one shot "po shang feng"(tetanus). Actually I could waive this shot if I kept the record from the university in the US.
