1.查了一些舊帖，大概發現需要填的表格如下：
I-130
I-485
G-325A
I-864
I-693

請好心的人幫忙看一下，是否齊全？

This is for your Mom. Your Dad only needs I-130. You also need I-131 if you Mom needs traveling. I-693 will be filled by physician so you don't need to do it yourself.

2.父母及本人需要準備哪些材料，比如說出生證.....之類的文件？

Read I-130 instructions, very clear there.

3.父母需要各填一張表嗎？

Yes.

4.所有的表格都要準備好一起寄過去嗎？包括體檢的表？

Yes, send to Chicago lockbox address for your Mom. For Dad, read I-130 instruction -- depending on where you live.
