Each criteria should be separeted by a colored sheet and print the criteria on the to of the colored sheet, then list exhibit one by one:

Awards: certificate, committee letter, level (evidence)
Membership: .....
Citations: Pubmed, ISI, google scholar
Your works impact your area: Review paper cited your work (line by line, highlight), medium reported your work.
Publications: Journal home page, scope, etc.
Review paper: all email communications (invitation email, agree review email, email, thanks email), your review sheet. It should be better add the journal information too (home papge, impact, scope).
