question about last entry form 485

I entered the USA in 1998 as F2 status. From then until now , I changed my status from F2-F1-H1. and changed my citizenship from Chinese to Canadian(I went to Canada for a lot of times). After I got my Canadian citizenship, I was refused to enter to USA(h1status) because I did not take my I-94 with me. I was able to enter the USA again after I brought my I-94 with me.
My question is, in form 485, Part 3. what I should fill in

Place of Last Entry Into the United States(City/State)

In what status did you last enter

Were you inspected by a US Immigration office

Nonimmigration Visa Number

Date visa was issued
