I did the medical exam 2 yrs ago, at that time, doctor skip the TB skin test and have the X-ray done, negative. But just last month, I got a RFE, specially state that I have to do TB skin test and if positive, another X-ray!! SO, do not skip the TB skin test.

This doesn't make any medical sense, but it is a new rule change at CIS. Just follow it.
