I would suggest you:
Each disctrict court has slight different rules, thus the template, even format might be different. Also filing through attorney vs Pro Se might be a little different too.
So first thing first, get your court rule, decides pro se or through attorney. Search my older post for details.
1. Open an account in PACER (this is free, need credit cards though).
2. Login to your local distruct federal court.
3. Find from there, cases represented by lawyers. I do not know if there is copy-right issue there or not, but you got the point. Find a good lawyer who actually have done a lot of litigation, and have "high" success rate, hire that lawyer, or "borrow" the format (template), and prepare yours from there.
4. Do keep in mind, you are suing a monster, and it has to be serious. Do not think all you lose is $350.00 if you can not prevail. THIS IS SERIOUS BUSINESS.
5. Home work, home work, home work...
It is not easy since early this year, but justice still exists, so if you are serious, your chance of win will be high.
