If you adjusted your immigration status without leaving the U.S., the I-94 slip on each approval notice should bear the same number as what is on your "Real Card".
I remember when I filed my application, I just copied all of these I-94 cards and included them in my application package. I wrote besides each I-94 record for the status changed (Original Entry, F1-H1, H1-H4, etc.)
I did not receive any question or challenge from the USCIS about this part of my application.
