TB test detail

I have recently received a RFE for TB test, obviously we didn't do it last time, and now the CDC rule changed and you cannot waive it. That's fine.

But when I called the doctor's office, they say only the nurse will do the test and sign the form (from CIS). But in the CIS notice, it says the civil surgeon must sign and seal the envelop. So what should we do ????

Any one with recent similar experience??
