Xiaobaitu and other DAXIA: please advise on my H4 extension

Hi. I just filed my I485 petition. My hu*****and will transfer his H1b. I want ot join his petition (H4 extension). In I-539 APPPLICATION FORM, PART 4 3a-c (have you ever apply an immigration visa), if I choose "Yes", will it affect H4 extension? BTW, do I need to enclose a seperate sheet of paper to explain (eg. my application number - I have not received one yet)? Any advice will be appreciated! Thank you!!
