I got it from Toronto Consulate in Sept. 2005. No need to apply for M-entries. That's the benefit for Canadian PR. You are automatically eligible for that once you become a landed PR. You don't have to have to have the maple card. The landing paper will do the work. You can double check with the visa officer upon receiving notice that your application is approved at the interview. I asked the VO in particular and he confirmed to me on site. Good luck.
