Even if it is a mistake, 2004-4-1 will still be your last day allowed to stay. You should 1. go to local immigration office to correct that, or 2. re-entered from Canada again asking for a new I-94 (You need a valid H-1 visa stamp in order to get a new I-94 in this case), or 3. file I-102 with service center. You MUST do one of the above before April 1, 2004. 1 better than 2 better than 3.
