Just for your reference:

1. Yes, you can see the red number in your visa page inside your passport.
2. Do you have your H1 visa stapmed on your passport? if not, you should fill the date of your F visa. Reminder, H1 status is different from H visa. Visa must be stamped on your passport.
3. No idea, maybe it count in.
4. I think file number is A#, if you don't have, fill in N/A.
