What docs needed when using EAD to work?

I am using H1b (school) working as a postdoc now. My NIW 140 has been approved, 485 is pending. My current postdoc will end this June and planning to work using EAD in industry. I have a few questions hope you folks can help.

1. Do I need to file any paper work to USCIS in order to work in industry using EAD based on pending 485?

2. What ususally an employer will ask for authorization to work? Just the physical plastic EAD card?

3. My wife is using her EAD to work now after using AP to enter US. Her I-94 has an expired parol date (Jan, 2007). Will there be any problem?
